We accept all major insurances in Illinois, including Medicare, Medicaid, private insurance, commercial insurance, and self-pay patients for in-clinic and home services.
Homebound Requirement: To qualify for our home healthcare services, you must be primarily homebound or unable to leave home unassisted. Being homebound means that leaving home requires significant effort and is taxing to your health.
Remote Patient Monitoring (RPM): RPM services are available for patients with one or more chronic conditions that require continuous monitoring. Eligibility depends on your insurance plan.
Primary Care Management (PCM) / Chronic Care Management (CCM): Patients with chronic conditions active for over a year and requiring healthcare professional attention more than once a month may be eligible for PCM/CCM services, depending on insurance coverage.
Diagnostic Imaging Services: Diagnostic imaging, including X-rays, ultrasounds, and EKGs, can be performed on-site at the clinic or at home, depending on your insurance.
Other Services: All other services, including medication management, wound care, mental health, pain management, Podiatry, and preventive care, are available both in-clinic and at home, based on your insurance coverage.
